Patient Intake
- As you set up your first appointment with our main office, you will be sent links to our electronic patient portal to fill out your intake paperwork in a paperless form.
- Paper intake forms will be provided only by prior request for those with special needs.
- Please fill out the provided online forms at least two days prior to your appointment.
- Any new patient without signed and returned intake paperwork will be rescheduled.
- Please read through the list of Office Policies below before your appointment.

- No medical information or suggestions will be made outside of an established appointment to insure continuity and quality of care.
- No patient information can be communicated via text or email as per HIPPA guidelines.
- All healthcare information must be provided via your Patient Portal.
- For continuity of care within established medical requirements, we cannot interpret any results unless provided through your patient portal, via fax from another provider, or
- Please makes sure you have "SHARED" all upload files with your provider (HIPPA guidelines)
Appointments:
- Functional Wellness Initial Visits or Re-activated patients (>1 year from last visit) will require a 1.5 hour consultation.
- Initial visits for patients within the local area are preferred to allow for physical examination.
- Functional Wellness Follow up visits are 30 min without new data, or 1 hour with laboratory findings (whether internal or externally supplied).
- We will attempt to run as close to schedule as possible, but may run behind in cases of clinical necessity. We appreciate your understanding.
- If you or your dependent requires a school or work exemption form, please request at time of service.
- For the courtesy of our other patients, any patient arrival more than 10 minutes late from the starting time of your appointment will need to reschedule.
- Please make any rescheduling requests at least 24 hours to your appointment.
- We reserve the right to bill patients for any skipped appointment without prior notice. (No Show Policy)
- Please arrive at least 20 minutes prior to any initial appointment, or 10 minutes prior to any followup appointment.
- We reserve the right to expand billing based on time utilized if your appointment requires additional time beyond your allotted appointment.
Prior or Outside Medical Records:
- Please upload all prior or outside medical records or medical information to be considered at least two days prior to your appointment. If you have physical records that need to be scanned in, please make sure they are mailed to our office or dropped by at least two days prior to your appointment.
- No records presented on the day of your appointment will be included in your evaluation, to insure the quality of their interpretation.
- No records will be read off of your personal phone or devices during your evaluation or appointment.
Laboratory Findings:
- Laboratory or Imaging findings are presented at the time of a Report of Findings consultation. This is to ensure information is presented in a clear fashion along with an interpretation and to ensure quality and continuity of your healthcare. A release without interpretation or consultation can constitute abandonment under medical ethics.
- Exceptions can be made on an individual basis with prior arrangement, where release is required due to medical emergency, or is needed for an emergent medical consultation or treatment with another provider.
- Release of medical findings can also be done with 30 days notice, according to HIPPA guidelines, if you are seeking followup with another provider.
Billing:
- Superbills - must be request on the day of service. Super bills for are provided as a courtesy, but our office operates on a strictly direct-pay basis.
- No Insurance is accepted. We apologize, but the current medical insurance model does not directly bill a functional nutrition or natural medicine approach.
- Year-End Billing summaries are not provided.
Supplements
- Our office recommends specific, practitioner quality supplementation. Patients are strongly encouraged to buy your supplementation directly through our office, or via our online dispensary.
- Online supplements, such as through Amazon or another unregulated vendor, have been shown in multiple cases to be counterfeit and we cannot guarantee their quality. Please be careful about your supply, as unregulated supplements can put your health at risk.
Returns:
- Once processed, laboratory testing orders are non-refundable due to the nature of the order processing by our third party laboratories.
- Supplements purchased directly from the office may be returned in an unopened state within 30 days of purchase.
- Opened supplements are non-refundable.
