Most new patients begin in one of three ways: with an initial consultation, with a testing-focused inquiry, or with a brief meet-and-greet to learn more about the practice.

I Have a Specific Health Concern

If you have ongoing symptoms, a chronic health concern, confusing lab results, or a problem that has not resolved with previous care, an initial consultation is usually the best place to begin.

During this visit, your provider will review your history, symptoms, prior records, current concerns, and goals. From there, we can help determine whether additional testing, nutritional support, chiropractic care, acupuncture, Hydrowave therapy, or another service may be appropriate.

​I Want Testing or Lab Interpretation

If you are interested in functional or advanced testing, we can help determine which options make sense before moving forward. Patients commonly ask about food sensitivities, gluten evaluation, candida testing, heavy metals, hormone patterns, metabolic markers, or a more detailed review of conventional bloodwork.

In some cases, testing can be arranged before your first full provider consultation so that your initial visit can focus on interpretation, clinical context, and next steps.

If you are new to functional wellness care, unsure which provider to see, or simply want to understand our approach before scheduling a full visit, you may request a brief meet-and-greet.

This is a short informational visit designed to help you get to know the practice, ask basic questions about services, and decide whether Lakeline Wellness Center is a good fit for your needs.

New Patient Consultation

A new patient consultation is designed to give your provider time to understand your case in context. Rather than looking at one symptom or one lab value in isolation, we review your history, current concerns, prior testing, lifestyle factors, medications, supplements, and health goals.

For functional wellness patients, initial visits are scheduled for a longer appointment time so that your provider can complete a more thorough review and begin building a practical plan. When appropriate, this may include recommendations for additional testing, nutritional strategies, supplement support, lifestyle changes, referral considerations, or follow-up visits.

Local patients are generally encouraged to schedule an in-person initial visit when possible, especially when a physical examination may be helpful.

Meet-and-Greet Option

A meet-and-greet is a short informational visit for patients who want to learn more before scheduling a full consultation. It is a good option if you are not sure which service is appropriate, whether our office is the right fit, or how functional wellness care differs from other models you may have tried.

Because this is not a full clinical appointment, it is not intended for diagnosis, treatment planning, lab interpretation, or detailed medical advice. It is simply a chance to ask basic questions and decide whether you would like to move forward.

Testing-First Pathway

Some patients already know they are looking for specific testing. Others have symptoms or prior lab findings and need help deciding which tests are most appropriate. Our office can help you understand available testing options and whether they should be completed before or after your first consultation.

Testing may include conventional bloodwork review, functional lab interpretation, food sensitivity testing, gluten-related testing, hormone assessment, metabolic markers, heavy metal or environmental exposure screening, and other specialty tests when appropriate.

Because test selection depends on the person, the symptoms, prior records, and the clinical question being asked, we recommend contacting the office before ordering or scheduling testing.

Patient Forms

After your appointment is scheduled, you will receive access to the electronic patient portal so you can complete your intake paperwork before your visit. Please complete your online forms at least two days before your appointment so your provider has time to review your information.

If you have prior lab results, imaging reports, medication lists, supplement lists, or records from another provider, please upload them through the patient portal at least two days before your appointment. This helps us review your case more thoroughly and prevents delays during the visit.

Paper intake forms are available only by prior request for patients with special needs.

What to Expect at the First Visit

Your first visit is a conversation, a review, and a planning session. Your provider will review your primary concerns, health history, prior records, current medications and supplements, lifestyle factors, and goals for care.

Depending on your needs, the visit may include a physical examination, review of previous lab work, discussion of testing options, nutritional or lifestyle recommendations, and a plan for follow-up. If new laboratory or specialty testing is recommended, your provider or the office team will explain the next steps.

For cases involving laboratory or imaging findings, results are typically reviewed during a dedicated Report of Findings consultation so they can be interpreted clearly and placed into clinical context.

What to Bring

What to Bring or Upload Before Your Visit

Please bring or upload the following before your appointment:

- A list of your current symptoms and main health goals
- Current medications and dosages
- Current supplements and dosages
- Recent lab results or imaging reports
- Relevant records from other providers
- A brief timeline of major diagnoses, surgeries, injuries, infections, or health changes
- Any specific questions you want to make sure are addressed

For best review, please upload records through the patient portal at least two days before your appointment.

 Insurance and Payment Expectations

Insurance and Payment Expectations

Lakeline Wellness Center operates on a direct-pay basis. We do not accept insurance directly for functional wellness or natural medicine services.

Payment is due at the time of service. If you would like a superbill for possible out-of-network reimbursement, please request it on the day of service. Reimbursement is not guaranteed and depends on your individual insurance plan.

Testing prices vary by test, and our office can help you understand pricing for specific assays before orders are processed. Once laboratory orders are processed through third-party laboratories, testing fees are generally non-refundable.

Mini FAQ

Common Questions

Do I need labs before my first visit?
Not always. Some patients already have labs or outside records to review. Others may need testing after the first consultation. If you are specifically interested in testing, contact the office so we can help determine the best starting point.

Can I do a phone or video consultation?
Video or phone consultations may be available for distance patients depending on the type of service needed. Local patients are generally encouraged to begin in person when a physical evaluation would be helpful.

Can I ask medical questions by text or email?
For privacy and quality-of-care reasons, healthcare information should be communicated through the patient portal once you are established as a patient.

What if I already have outside records?
Please upload them through the patient portal at least two days before your appointment so your provider has time to review them.
